SC AD Registry
The South Carolina Alzheimer’s Disease Registry is the oldest and most comprehensive
statewide registry of residents diagnosed with ADRD in the United States. The SC AD
Registry collects demographic and diagnosis-related information and provides prevalence
estimates of ADRD across the state. With formal permissions, the Registry data is
linkable to a variety of social and health-related datasets,. SC AD Registry is one
of three population-based studies supported by the Analysis Core.
Understanding the SC AD Registry

The SC AD Registry has maintained a record of diagnosed cases of ADRD in the state
of South Carolina since 1988. As of January 1, 2015 (the most current year data are
available), 95,899 South Carolinians were living with ADRD. These individuals are
identified from several sources when they (or their family members) seek provider
services, which ensures that the SC AD Registry captures as many diagnoses as possible.
The Registry comprises multiple data sources and is maintained by the Arnold School
of Public Health at the University of South Carolina. More information can be found

Access
For access to the SC AD Registry, including the Clinical Data Warehouse, CCADMR scientists
should log in to the Resources for Project Scientists portal on the CCADMR website
and complete the Application for Use of the SC AD Registry Data form. Other users
can request access to the SC AD Registry data using the data request form located
